THE HOUSING PROGRAM

Is a supportive program for persons whose major limitation is severe and persistent mental illness (SPMI) and are seeking supported housing.

EXTENDED SUPPORT CARE AND SUPPORTED LIVING HOMES

The purpose of the Extended Support Care and Supported Living Homes is to provide a structured group living experience to persons who seek assistance in developing daily living skills.

Services offer individuals in developing independent living skills as well as:

Limited transportation from Golden House

Staff supervision

Meals

Medication monitoring

TRANSITIONAL LIVING HOUSE

Location:  512 SW Western

This house deals with clients coming straight out of Osawatomie State Hospital. It is shared apartment living. Attendant care services are required for placement in Western house of at least 6 hours a day. The stay at Western house is limited to 6 months. At that time residents are moved to a more independent living situation. During their six months stay residents will be working on goals and daily living skills to be successful in the community.

SHARED LIVING APARTMENTS

The purpose of the shared living apartments is to provide semi-independent living to individuals in furnished, 2 bedrooms units. Individuals in this program are seeking minimal support.

Services offered in shared living apartments are community wrap around services.

Admission requirements:

Participants must be at least 18 years of age, and seeking to participate in a program with supports through case management at Valeo, csscp program at V.A., or a utilizing Wellness call checks through Breakthrough, accept medical supervision and cooperate in an individually planned program of activities developed by residents and case manager’s.

1) material verifying past or present psychiatric treatment and proof of a SPMI.

Applicants will be notified of the decision and have the right to appeal all adverse decisions.
